SCOTT TANNER
FOREVER 28
March 22, 1988 - August 27, 2016
Scott attended his first rehab facility in Port Colbourne in September of 2012. He was able to maintain his sobriety for 5 1/2 months before relapsing. He continued to struggle with recovery even while attending numerous facilities in Ontario, including ones in North Bay and Windsor. In December of 2013, Scott made the decision to enter the rehab facility, '1835' in Alberta were he remained for almost 21 months in program.
In September of 2015 he relapsed once again and continued his substance use off and in over the next 11 months before entering the reputable rehab facility, Claresholm, located just outside of Calgary. There, a dual diagnosis program was offered to treat both addiction and the mental health side of drug addiction. Scott was released from that 3 month long program on August 26, 2016. Just 24 short hours later Scott would be found dead due a suspected heroin/fentanyl overdose.
